Required Admissions Forms
Several forms must be submitted by August 4th prior to enrollment. Click on the links below to access each form. The Health History Form should be mailed directly to the Thagard Student Health Center while the remainder of the forms should be returned to the Office of Admissions and Records at 425 West Jefferson Street, Tallahassee, FL 32306.
